Disclosure of Personally Identifiable Information

What Can I Do Without Disclosing Any Information?
Many of our services are available to you without entering any personal information: you can learn more about our company and how Medicare MarketSales works without providing any information. In order to actually purchase a policy, however, we require you to provide us with certain personally identifiable information.

Why Do I Need to Submit Personal Information?
Because coverage benefits and premiums for insurance policies are affected by your individual traits and circumstances, we ask you to disclose personally identifiable information when you purchase a plan online, by phone or by fax. Our goal is to allow you to quickly and easily purchase the insurance products that best fit your needs without jeopardizing your privacy and security in any way. We ask you to disclose personal information in order for us to:
• Provide you with the insurance policies and/or protection plans that best meet your unique needs and preferences.
• Verify that your applications and/or purchase orders are received and accurately processed by the appropriate insurers and/or plan administrators.
• Maintain accurate records about who you are and where you learned about us.
• Maintain accurate policy records and a personal profile for your secure access via the Web.
• Deliver important materials to you pertaining to your selected policies and/or plans.
• Contact you and/or your designated contacts directly should the need arise.

What Kinds of Information Will I Have to Provide?

Personal Information
This may include your name, address, date of birth, gender, telephone numbers, and Medicare information.
Other Personal Information Required for Coverage
Depending on the plan and coverage options you wish to purchase, additional information may be required about you and each additional person to be covered under the same policy or plan, or for whom you are purchasing a separate policy or plan. For each additional person to be covered you may be required to provide the same mandatory personally identifiable information you disclosed about yourself. You may be required to disclose your Social Security Number in order to purchase certain plans.
Health Information
Some insurance plans require you to submit certain personal information concerning your medical history in order to process your request. This includes, but is not limited to, your height, weight, and medical history (for diagnoses of heart condition, anemia, high blood pressure, diabetes, cancer, and other health conditions). We do not maintain records of your personal health information, nor do we make any other use of the information other than to pass it along to the insurer who will provide the policy to you.

If you begin an application for a plan that requires medical information, we will store your application in a secure location to facilitate completion of the application. If, after a reasonable time you have not completed the application or contacted us to keep your file open, that information will be deleted. We reserve the right to store your health information associated with specific policy applications for our records as necessary and as mandated by state and federal law. Any changes to the Statement of Privacy and Security will be posted immediately on this site, and you should always review the statement prior to submitting information.

What Do You Do with this Information?

We take the information that you provide and deliver it to the insurance provider/administrator that you have selected so that they can process your application.
Will Any of the Personal Information Collected be Disclosed to Outside Parties?
The insurance companies and plan administrators who administer your plan(s) will receive your information for the purpose of fulfilling your purchase order(s) and providing plan services. We do not divulge, sell, trade or rent your personal information to others. We may provide aggregate statistics about our customers, sales, traffic patterns, and related site information to reputable third-party vendors, but these statistics will include no personally identifying information.
How Do I Remove My Information from Your Database?
If you do not wish to receive marketing email from us, or if you decide you no longer wish to receive our newsletter, you can remove yourself from our distribution lists at any time by:
sending an email request for removal to info@insurcenter.com or by writing to:
